ORLANDO, FL USA (UroToday.com) - Dr. Gwen Grimsby from Children’s Medical Center of Dallas, TX presented a series of patients with coronal fistula and well-fused glans wings who underwent reoperation with elevation of the glans instead of performing another TIP urethroplasty. Forty-five fistulas that were < 3mm in size were repaired in this manner. The first 17 had a 6F catheter placed for urinary diversion, but the next 20 patients in the series did not. Two fistulas recurred over 18 months of follow-up; 1 occurred in the group that was diverted and 1 in the group without diversion.
Thus, small coronal fistulas with well-fused glans wings (not simply a skin bridge crossing the glans) do not require full redo TIP urethroplasty.
