Approximately 25% of patients with penile squamous cell carcinoma (PSCC) without clinically palpable inguinal lymph nodes (cN0) harbor micrometastatic disease that is undetectable by conventional imaging. Compared to delayed excision, early excision of lymph node (LN) metastases improves cancer-specific survival among patients with PSCC (84% vs 35% at 3 yr). Both the European Association of Urology and the National Comprehensive Cancer Network (NCCN) guidelines recommend inguinal LN dissection (ILND) or dynamic sentinel LN biopsy (DSNB) for patients with intermediate- or high-risk PSCC (≥pT1G2) with cN0 inguinal basins.
