ORLANDO, FL USA (UroToday.com) - During the “Advanced Prostate Cancer II” session, Dr. Matthew Cooperberg presented data from PROCEED, an ongoing Phase 4 registry of patients with metastatic castration-resistant prostate cancer (mCRPC) treated with sipuleucel-T (ST).

Since sipuleucel-T approval in 2010, the treatment landscape for metastatic castration-resistant prostate cancer (mCRPC) now includes the androgen-receptor signaling pathway inhibitors (ASPIs) abiraterone acetate or enzalutamide.

Vaccine approaches for cancer differ from traditional vaccine approaches for infectious disease in tending to focus on clearing active disease rather than preventing disease. In this review, we provide a brief overview of different types of vaccines and adjuvants that have been investigated for the purpose of controlling cancer burdens in patients, some of which are approved for clinical use or in late-stage clinical trials, such as the personalized dendritic cell vaccine sipuleucel-T (Provenge) and the recombinant viral prostate cancer vaccine PSA-TRICOM (Prostvac-VF).

The tumors of many patients with prostate cancer eventually become refractory to androgen deprivation therapy with progression to metastatic castration-resistant disease. Significant advances in the treatment of metastatic castration-resistant prostate cancer (mCRPC) have been made in recent years, and new treatment strategies have recently been made available.

SEATTLE, WA USA (Press Release) - September 17, 2013 - Dendreon Corporation (Nasdaq: DNDN) today announced that the European Commission (EC) has granted marketing authorization for PROVENGE® (autologous peripheral blood mononuclear cells activated with PAP-GM-CSF or sipuleucel-T) dispersion for infusion in the European Union (EU) for the treatment of asymptomatic or minimally symptomatic metastatic (non-visceral) castrate resistant prostate cancer in male adults in whom chemotherapy is not yet clinically indicated.

Following FDA approval of sipuleucel-T in 2010 for metastatic castration resistant prostate cancer (mCRPC), several studies have described the effect of sipuleucel-T on peripheral immune responses. Retrospective associations have also been made with immune responses and survival.

ORLANDO, FL, USA (UroToday.com) - In this study, as of September 14, 2012, 7 patients received an infusion of sipuleucel-T. These patients had progressed to mCRPC and had been previously treated with at least one infusion of sipuleucel-T on the PROTECT trial. The median interval since the last infusion had been 8.6 years. This is the first study to demonstrate an immunological memory response of several years after initial treatment in patients being retreated with sipuleucel-T. The retreatment was well tolerated and the antigen-specific cellular responses were boosted after retreatment.

Optimal sequencing of ADT and sipuleucel-T to maximize immune and clinical responses is unknown.

ORLANDO, FL, USA (UroToday.com) - In this session, Dr. Antonarakis presented interim analysis from a Phase II trial evaluating sequencing of ADT and the immunotherapy “vaccine” sipuleucel-T in 68 men with biochemically-recurrent prostate cancer who were randomized over nine months. Sipuleucel-T and ADT were both well tolerated in these patients.

ORLANDO, FL, USA (UroToday.com) - The Phase 3 IMPACT study showed significant improvement in overall survival with sipuleucel-T immunotherapy. The PROCEED trial is an ongoing Phase 4 registry enrolling patients receiving sipuleucel-T in the real-world setting. Enrollment has no restriction on whether docetaxel has been used to treat.

In recent years, immunotherapy has emerged as a viable and attractive strategy for the treatment of prostate cancer.

Cancer vaccines, cytokines, and checkpoint inhibitors are immunotherapeutic agents that act within the cancer immunity cycle. Prostate cancer has provided unique opportunities for, and challenges to, immunotherapy drug development, including low tumor mutational burdens, limited expression of PD-L1, and minimal T-cell intratumoral infiltrates.

Multiple immunologic platforms have provided minimal impact in patients with metastatic castration-resistant prostate cancer, necessitating that novel approaches continue to be developed. Although checkpoint inhibitors have been largely ineffective, there remain small cohorts of patients who have durable responses but lack the conventional indicators for response to this class of drugs, that is, high mutational burden or significant genomic alterations, as seen in other solid tumors.

Treatment of cancer patients involves a multidisciplinary approach including surgery, radiotherapy, and chemotherapy. Traditionally, patients with metastatic disease are treated with combination chemotherapies or targeted agents.

Immunotherapy is rapidly transforming cancer care across a range of tumor types. Although Sipuleucel-T represented the first successful vaccine for the treatment of established cancer, other immunotherapeutic approaches for prostate cancer such as checkpoint inhibitors have been relatively disappointing to date.

Prostate cancer was the first cancer in which immunotherapy vaccine was approved by FDA in 2010 named Sipuleucel-T. No new immunotherapies have been approved since, as phase 3 trials didn't show any improvement in overall survival (OS) especially with immune checkpoint inhibitors.

The management of advanced prostate cancer, specifically metastatic castrate-resistant prostate cancer (mCRPC), remains a therapeutic challenge. Sipuleucel-T (Provenge; APC8015) was approved by the FDA in 2010 for the treatment of asymptomatic or minimally symptomatic mCRPC patients, and it remains the only FDA-approved immunotherapy for prostate cancer of any indication to date.

In this review, we aim to describe racial differences in response to treatment for metastatic castration-resistant prostate cancer (mCRPC).

Recent data suggests that, despite higher risk features, African Americans may respond better than Caucasians to systemic therapies for advanced prostate cancer.
