Bladder injury during rectal surgery- a video vignette.

We present a case of an intraperitoneal bladder injury during laparoscopic rectal surgery for pelvic organ prolapse. Lysis of adhesions was necessary to access the pelvis. We used an ultrasonic dissector and inadvertently caused damage to the bladder dome. Various risk factors increase the chance of bladder injury during abdominopelvic surgery: these include previous surgery or adhesions, bladder diverticula, malignancy or any other procedure causing anatomical distortion or inflammation. Population-based estimates of bladder injury for gastrointestinal surgery have been estimated at 0.15%. They may be intraperitoneal or extraperitoneal. Extraperitoneal bladder injury can be managed with an indwelling bladder catheter alone.. In contrast, intraperitoneal injury may result in urinary extravasation into the abdomen and operative repair may be necessary. This article is protected by copyright. All rights reserved.
